College of Engineering students, faculty, and staff may use this form to request off-hours access to various CoE rooms that are controlled by electronic locks. Your right to access the requested room will be verified before access is actually granted.

To complete this form, you will need your UK student, faculty, or staff ID card. Fields marked with an asterisk (*) are required.

16-digit LONG UKID number. Not all cards will have this number, but if your card does, you will need to provide it.
9-digit UKID number. This will be your UKID number plus the lead 9 on your ID card.
This is the number below the UKID number on your ID card. See the picture below for help finding it.

This is a 5 or 6-digit number on the bottom of the back of your ID card. There may be another digit and a "*" in front of it, and there is probably a longer number after it. It might be in a light brown or orangish color. See above if you need help finding this number.
ME Juniors and Seniors may request access to RGAN 312 while they are taking ME 411 and ME 412. Your enrollment in one of these courses will be confirmed before access is granted, and you must acknowledge that you have read and will abide by the lab rules for RGAN 312.
